How it works

What happens from your first call to the first shift

Our Home Healthcare is a nurse-led home care agency in Edgewater, Florida, and care starts within 48 hours of your call. Jordan calls you back within the hour. Priya or Dana visits the home within 24 hours, free, and you get a written plan and an exact price the same day. The first shift follows within 48 hours.

The first 48 hours

How fast can home care start in Edgewater

3 steps, and each one has a clock on it. A hospital discharge tomorrow is a normal Tuesday for us.

Step 1
1hour

You call, or send the form

Call (386) 272-6048 or send the form at the bottom of this page. Jordan calls you back within the hour, day or night. No script, no sales pitch, nothing to decide on the call.

Step 2
24hours

A nurse visits the home

Priya or Dana comes to the house within 24 hours. It is free, takes about an hour, and you leave it with a written plan of care and an exact price for it.

Step 3
48hours

Care starts

Jordan picks a caregiver for the person, not for the shift, and the first visit happens within 48 hours of your call. Jordan then calls you every week for the first month.

Step 1, the call

What does Jordan ask on the first call

Jordan Blake is our care coordinator and answers most of the calls to (386) 272-6048. In the day the phone rings in the Edgewater office. After 5 pm and on weekends it rings through to the on-call nurse. Either way a person picks up, and if you leave the form instead, Jordan calls you within the hour.

The call takes 10 to 15 minutes. Jordan asks 5 things.

  • Who the care is for and what is going on this week. A fall, a discharge, a diagnosis, or you are just worn out.
  • What help is needed. Bathing, meals, the pills, nights, company, rides, or you are not sure yet, which is fine.
  • Where the house is, so we know which caregivers live close by.
  • When you want care to start. Tomorrow is a normal answer.
  • When a nurse can come to the house. Jordan books Priya or Dana for a time within the next 24 hours, often the same day.

You do not need to know the hours, the budget or the diagnosis. You do not decide anything on the call.

Step 2, the free in-home visit

What happens at the free in-home visit

Priya Ramnath, RN, our nurse case manager, or Dana Whitfield, RN, the nurse who started the agency, comes to the house within 24 hours of your call. The visit is free and takes about an hour. She sits with your mother first, then with you.

What she looks at

How your mother gets out of a chair, into the shower and down the front step. Where the pills are and whether last week's are still in the bottle. What she eats, who cooks it, and what is in the fridge. How the nights go. The rugs, the bathroom, the lighting, the stairs. What her doctors have said and what has changed in the last month. And what you, the family, are doing now that you cannot keep doing.

What she writes

A written plan of care. Which days, how many hours, what the caregiver does on each visit, what to watch for, and when Priya comes back. The plan is yours whether you hire us or not.

What you leave the visit with

  • A written plan of care, in plain words.
  • An exact weekly price the same day, worked out by Tom from the plan and the published rates.
  • A nurse's view of what is safe in the house and what is not.
  • A start date, if you want one. Within 48 hours of your first call.
  • Nothing to sign on the spot. No deposit, no contract.

Step 3, care starts

How does the first shift work

Jordan matches a caregiver to the person, not to the shift. That means reading the plan, then choosing from our 64 caregivers by who lives close, who has done this kind of care before, and who your father is likely to let in the door. A quiet man who wants his coffee and the news gets a quiet caregiver.

The first shift happens within 48 hours of your call. Carla Jimenez, our lead caregiver, shadows every new caregiver on their first 3 shifts, so on a first visit there may be 2 people at the door. Jordan also picks 1 trained backup, who meets your father early so that a sick day never means a stranger.

What happens in the first month

Jordan calls you every week for the first month to ask what is working and what is not. Priya visits the house at least every 30 days to check on the plan and change it if she needs to. Tom sets you up on the Family Portal on day 1, so you can read the notes from every visit on your phone the same evening.

3

caregivers at most in the first 90 days. A primary, a backup, and Carla on the first shifts.

30

days at most between nurse visits. Florida requires 60. We do twice that.

4

calls from Jordan in the first month, 1 a week, to ask what to change.

Before the visit

What should you have ready for the nurse visit

None of this is required. The visit still happens if you have nothing to hand. But each of these makes the plan of care better and the price more exact.

Papers to have on the table

  • Every pill bottle, in a bag, including the ones she stopped taking.
  • The hospital discharge papers, if there was a stay.
  • The names of her doctors and the next appointment date.
  • A long-term care insurance policy, if there is one. Tom reads it for you.
  • A VA discharge paper (DD-214) if your father or his late wife served.

Things to think about before she arrives

  • Which days and times are hardest right now. Mornings, nights, weekends.
  • What your mother will and will not accept help with, in her own words.
  • Who else helps now. A neighbor, a church friend, a sister who comes on Sundays.
  • Whether the person wants to be in the room for the whole visit. Most do.
  • Your questions. Write them down. Priya will answer every one before she leaves.

If it goes wrong

What if it is not working

Sometimes the match is wrong. Sometimes the need changes. Sometimes a family tries care and decides it is not for them yet. Here is what happens in each case.

1

The caregiver is not the right person

Tell Jordan. We change the caregiver within a week, no questions asked and no explanation needed. The backup caregiver, who already knows the house, usually covers the gap. In the first 90 days you still meet at most 3 caregivers.

2

The hours are wrong

Call Jordan and the schedule changes from the next week. More hours, fewer hours, a night added, a morning dropped. The price changes with the hours. Priya rewrites the plan if the need has changed.

3

You want to stop

Give us 48 hours notice and care stops. There is no contract, no deposit to lose and no fee. You pay for the hours used. If you want to start again in 3 months, you call Jordan and we start again.

What families say

Priya was at the house Friday morning

Dad came home from Halifax on a Thursday and Priya was at the house Friday morning sorting his pills into the box. He has not been back since.
Mark, Port Orangeson
I called at 11 at night because Bill fell. A nurse answered. Not a service, a nurse.
