Our Home Healthcare is a licensed home health agency in Edgewater, Florida, and hospital to home is our 30-day program for anyone leaving a hospital or rehab stay. A nurse visits within 24 hours of discharge, sorts the medications, books the follow-up appointments, and a caregiver covers the first nights. $2,400 flat for the 30 days.

You cannot be there Thursday night. A caregiver like Marcus can. He covers the first nights, Priya is at the house Friday morning with the discharge papers in her hand, and Jordan calls you every week for the first month.
Rehab is ending and your mother is not ready to be alone
She can walk to the bathroom with the walker and not much further. 40 caregiver hours in the first month cover the mornings, the showers and the rides to the follow-up appointments, and the plan changes as she does. Results vary from person to person.
What we do
What happens in the 30 days after discharge
The program has 4 parts and 1 price. This is the order they happen in.
- Day 1
- Priya is at the house within 24 hours of discharge. She reconciles the medications (the discharge list against the kitchen counter), fills the weekly organizer, and checks the wound, the weight or the numbers the hospital was watching.
- Night 1
- A caregiver for the first nights, awake if needed, so nobody gets up alone at 3 am.
- Week 1
- Every follow-up appointment on the discharge papers booked by Jordan and driven to by the caregiver. 40 caregiver hours in the month, used where they are needed.
- Visits 2 and 3
- 2 more nurse visits before day 30, 3 in all. Priya checks the medications again, looks at what has changed, and writes the plan for month 2 if there needs to be one.
After day 30 many families keep the caregiver on for personal care at $32 an hour, or keep Priya coming for skilled nursing visits at $95. Some need neither. If your father is coming home on hospice, that is comfort care on the all services page, and hospice brings its own nurse.

What it costs
How much does hospital to home cost in Edgewater
$2,400 flat for the 30 days, agreed before anyone comes to the house. Hours beyond the 40 are $32 an hour and extra nurse visits are $95. No deposit, no contract.
Private pay by card or bank draft, billed weekly. Long-term care insurance, and Tom Whitfield bills the insurer for you. VA Aid and Attendance for veterans and surviving spouses, and we help with the application. We do not take Medicare or Medicaid.

Jordan calls you back within the hour, day or night. You tell us what is going on. No script, no sales pitch, and no decisions needed on the call.
A nurse visits the home
Priya or Dana comes to the house within 24 hours. It is free, takes about an hour, and you leave it with a written plan of care and an exact price for it.
Care starts
Jordan picks a caregiver for the person, not for the shift, and the first shift happens within 48 hours of your call, or on the night of the discharge if that is sooner. Jordan then calls you every week for the first month.
